When you enroll, Illizeo asks Who do you want to cover?. You tick the relatives to include, and the coverage scope follows from your selection. You are covered in every case.
The four coverage scopes #

The Coverage summary shows the resulting scope and the number of people covered, including you, before you confirm.
Why it matters #
Each scope has its own price: covering a whole family does not cost what covering one employee costs, and the insurer bills them differently. Your contribution is calculated on the scope actually selected.
No relative to tick? #
Only dependents declared in your profile can be covered. If the list is empty, add them first, then come back and enroll.
